East Bay Community Action Program is hiring for a Case Manager, Older Adults and Adults with Disabilities for our 100 Bullocks Point, Riverside, RI location. The Case Manager is responsible for providing person-centered, unbiased care coordination and case management services to aging and disabled (AED) population, as well as to adults living with intellectual and developmental disabilities (I/DD). This role involves assessing client needs, developing Person – Centered care plans, coordinating community services, and ensuring comprehensive documentation. The Case Manager works closely with clients, families, and interdisciplinary teams to promote client well-being, maintain compliance with regulatory guidelines, and ensures that all services are delivered in a respectful, confidential, person - centered and culturally competent manner.
Responsibilities:
- Coordinate care and services for aging and disabled (AED), and adults with intellectual and developmental disabilities (I/DD)
- Conduct comprehensive assessments of clients’ needs, strengths, and resources.
- Conduct client visits and meetings in various settings such as homes, assisted living facilities, and group homes, while adhering to agency safety protocols.
- Develop, implement and monitor person - centered care plans, addressing medical, social, and community service needs.
- Coordinate the provision of services with direct service providers such as Home Care agencies, Adult Day Health centers, and I/DD service providers.
- Provide ongoing case management and respond to clients’ inquiries.
- Become proficient in electronic case management systems and submit all required documentation in compliance with the State regulations.
- Connect clients to additional services and supports.
- Ensure compliance with Conflict - Free Case Management (CFCM) regulatory standards and documentation requirements for Cost Share and Medicaid AED and I/DD clients.
- Investigate and address Adult Protective Services (APS) cases, ensuring the safety and well-being of vulnerable clients.
- Report abuse, neglect, or exploitation cases to appropriate authorities.
- Maintain confidentiality and provide culturally competent, person-centered services
- Other duties as assigned.
Qualifications:
- Associate’s Degree in a Social Services field with relevant experience required.
- Bachelor's degree in Counseling, Education, Gerontology, Human Services, Nursing, Social Work, Psychology, or a related field is preferred.
- At least two years of relevant experience working with low-income elders and individuals with disabilities.
- Valid driver's license, automobile insurance, and reliable transportation required.
- Willingness to travel throughout Rhode Island.
- Knowledge of human behavior and the aging process.
- Ability to work effectively with people from diverse cultures and backgrounds in a supportive capacity.
