Community Tx Specialist-Residential Case Manager

$2,000 Sign on bonus

East Bay Community Action Program (EBCAP) has an immediate full-time, Community Tx Specialist/Residential Group Home Case Manager for our residential group home in East Providence and Warren, RI that serves up to eleven (11) adult men and women.

The individual will assist psychiatrically disabled residents of a Residential Rehabilitation Facility with learning and increasing daily living skills and structure, assisting clients in individual growth and optimizing personal independence, while adhering to individual treatment plans.

Responsible for medication monitoring and conduct assessment of individual cases to determine clients’ needs in regard to basic life areas and adult daily living skills, including, but not limited to, food, shelter, hygiene, medication, transportation, health care, financial management, as well as decent, safe, and sanitary living conditions. Provide a variety of services that educate, teach, supervise, assist, and encourage clients to attain the highest possible levels of independence and safety. Coordinate clinical care of assigned clients with multi-disciplinary team, as well as directly with the clients, and family members (if applicable), to ensure assessed needs are met. Immediately report to supervisor any conditions that fall outside the norms of acceptable situations, such as unsafe or unsanitary living conditions, decline in client functioning, etc.

Educate and assist clients regarding household management, daily structure, adult daily living skills (ADL), nutrition, and food safety.

Minimum Education Requirements

Associate's degree in a Human Services field required. Bachelor's Degree (BA) degree in Human Services preferred. Commensurate years of direct care experience may be considered in place of an educational degree. Must have a valid driver’s license and good driving record if engaged in travel on behalf of the organization.
