East Bay Community Action Program (EBCAP) is seeking a full-time Case Manager to join our Behavioral Health Services Division as part of the Assertive Community Treatment (ACT) Team. The ideal candidate will hold a Bachelor’s degree and be a Licensed Chemical Dependency Professional (LCDP). This position involves providing comprehensive case management and care coordination services to support individuals living with Severe and Persistent Mental Illness (SPMI) who are residing independently in the community.
Key Responsibilities:
Provide case management services and substance use disorder treatment in the office, community, and clients' homes.
Support clients in identifying triggers, developing relapse prevention strategies, and building strong support systems.
Educate clients on the physical, medical, and pharmacological aspects of substance use.
Apply ASAM criteria to determine the appropriate level of care and guide treatment planning.
Administer urine toxicology screenings and breathalyzer tests as required.
Document all client encounters in the electronic medical record within 48 hours.
Complete all necessary clinical documentation, including treatment plans and annual assessments, in compliance with licensing and regulatory standards.
Meet established productivity standards.
Maintain a reduced caseload, as required for this position.
Assist clients with scheduling and attending medical, dental, and vision appointments, providing one-on-one support as needed.
Help clients access and apply for state and federal benefits, including but not limited to SSI, SSDI, SNAP, and Medicaid.
Support clients in securing and maintaining safe, affordable housing.

Qualifications:
Bachelor’s degree in a Human Services field and a minimum of two (2) years of direct experience required
Licensed Chemical Dependency Professional (LCDP) credential required
Must possess a valid driver’s license, an acceptable driving record, and reliable transportation with current insurance
Travel is required as part of this position to fulfill organizational responsibilities
Excellent interpersonal and communication skills
Ability to work effectively both independently and collaboratively as part of a team
