Community Treatment Specialist - Full Time Position

Location: 2 Old County Road, Barrington, RI
Division: Behavioral Health Services

EBCAP is seeking a full-time Community Treatment Specialist to join our multi-disciplinary Assertive Community Treatment Team. This rewarding position offers the opportunity to provide essential support services to clients in our community.

What You'll Do

Key Responsibilities:

• Perform comprehensive case management and care coordination duties on our Community Support Team
• Provide direct assistance, advocacy, coordination of care, and individualized support to clients
• Assist clients in scheduling and attending medical, eye, and dental health care appointments
• Document all encounters in the electronic medical record within 48 hours
• Complete required clinical documentation including Treatment Plans and Yearly Assessments to meet licensing and regulatory standards
• Meet established productivity standards
• Help clients access state and federal entitlements including SSI, SSDI, food stamps, and Medicaid
• Assist clients in obtaining and maintaining safe and affordable housing

Schedule

Primary Hours: Monday-Friday, with rotating weekends and holidays.

Monday- Friday 8:00 AM to 4:30 PM with on evening shift until 6pm

weekend and holiday shifts are 8:00am-12:00 pm

What We're Looking For

Qualifications:

• Bachelor's Degree in Human Services field and experience preferred
• Valid driver's license, acceptable driving record, and reliable transportation with insurance (travel required)
• Strong interpersonal and communication skills
• Ability to work independently and as part of a collaborative team
