CALLING ALL HEALTHCARE HEROES! East Bay Community Action Program is seeking an exceptional Primary Care Provider to lead our integrated healthcare approach!

You'll provide comprehensive medical services to individuals and families across all ages while pioneering an innovative integrated care model that synchronizes with mental health, dental, and social services under one roof.
COMPREHENSIVE PATIENT CARE • Examine, diagnose, and treat patients across the lifespan using state-of-the-art medical equipment • Document patient histories in our electronic medical record system • Order diagnostic tests, analyze results, and develop treatment plans • Prescribe medications and administer treatments including vaccinations • Provide specialized care including prenatal/postnatal services • Educate patients on preventive health measures and lifestyle modifications • Make strategic referrals to specialists when advanced care is needed
LEADERSHIP & TEAM INTEGRATION • Supervise and mentor allied health professionals including nurses, physician assistants, and medical assistants • Collaborate seamlessly with our Integrated Care Coordination Team • Connect medical services with EBCAP's comprehensive social services network • Lead by example in our innovative care delivery model
QUALITY EXCELLENCE • Ensure the highest standards of clinical care delivery • Maintain detailed records supporting quality patient outcomes • Participate actively in professional development activities • Partner with our Medical Director on continuous improvement initiatives
Education & Credentials: • MD from accredited medical school, OR • DO from accredited osteopathic medical school, OR • Master's degree in Physician Assistant Studies, OR • Master's/Doctoral degree in Nursing with Family Practice specialization
Licensure & Certifications: • Current Rhode Island state license (MD, DO, PA-C, or NP) • Board certification in Family Medicine/Family Practice • Current DEA registration • Basic Life Support (BLS) certification • Advanced Cardiac Life Support (ACLS) preferred
Clinical Expertise: • Completed residency or equivalent clinical experience • Proficiency in neonatal/infant care, women's health, and pre/post-operative care • Experience treating conditions across all age groups • Knowledge of psychiatric disorders and community health issues
• Unwavering commitment to patient confidentiality and ethical standards • Cultural sensitivity and ability to provide respectful, inclusive care • Collaborative spirit to work across EBCAP's integrated service model • Adaptability in addressing health disparities in diverse populations • Comfort with AI technology for enhanced documentation • Advocacy for inclusive healthcare practices
• Multilingual capabilities (Spanish/Portuguese highly valued!) • Experience serving diverse populations • Training in culturally responsive healthcare delivery
